It may be helpful to give your doctor this prescription form to fill out. It contains all the information required by your insurance and can save us from having to contact your doctor to get a missing piece of information. Try to leave the doctors office with this prescription in your hand and you can send us a photo. Make sure they fill out all sections. If your doctor’s office insists on sending it themselves, have them fax it to us at 844-781-8303 or email it to office@bigislandmassagenetwork.com.
Hawaii state law requires that the prescription be signed by a doctor with MD or DO credentials. However, due to the doctor shortage, many doctors in Hawai’i are actually nurse practitioners (DNP or APRN credentials). If you are going to urgent care, make sure there is a MD or DO on staff that day that can sign your prescription before you wait a long time to be seen. A few insurance companies may allow a prescription from an APRN, but don’t count on that unless you verify that first.
